Prevalence of IgA anti-HLA antibodies and previous graft survival in kidney re-transplant candidates

Background
Modified bead array assays allow the detection of immunoglobulin subclasses including IgA. Previous data showed that IgA antibodies can readily be detected in nearly half of the sera of re-transplant candidates. They seemed to occur independently of IgG antibodies and displayed donor specificity in half of the cases. However, the relevance of IgA HLA-antibodies remains unclear. Data from our group indicate that re-transplant candidates with IgA antibodies had a shorter graft survival of their previous kidney transplant. As surrogate marker the “back on waiting list time” can be used as end point for the survival of graft.
Objective
To determine the prevalence of donor specific IgA anti-HLA alloantibodies in the serum of kidney re-transplant candidates and to assess previous graft survival in association with the presence or absence of IgA alloantibodies.
Work plan
Serum from re-transplant candidates, e.g. kidney transplant patients with at least one rejected transplant who are listed for re-transplantation will be analysed by modified bead array assay for the presence of IgA anti-HLA class I and II alloantibodies. 300 microliter of serum would be appreciated. To determine donor specificity information of recipient and donor typing is required, as well as the latest alloantibody status as it had been determined in the local routine testing. The crucial information will be the time of the last transplantation and the back on waiting list time (BWLT) - alternatively the time between last transplantation and graft dysfunction - as indirect marker for graft survival,. This information should be provided in a separate spreadsheet.
Costs
No charges for participants will ensue, apart from mailing expenses.
